Quick answer: Zepbound starts at 2.5 mg weekly for 4 weeks, then 5 mg for 4 weeks. From there, doses can step up every 4 weeks: 7.5 mg → 10 mg → 12.5 mg → 15 mg (ceiling). Most prescribers slow the schedule if side effects are intense.
Standard schedule
Eli Lilly's FDA-labeled schedule is 4 weeks per step. Real-world practice often extends a step when side effects are heavy or when a lower dose is producing adequate weight loss.
- Weeks 1–4: 2.5 mg (starter — not for maintenance)
- Weeks 5–8: 5 mg
- Weeks 9–12: 7.5 mg (optional)
- Weeks 13–16: 10 mg
- Weeks 17–20: 12.5 mg (optional)
- Weeks 21+: 15 mg (ceiling, only if needed)
When to slow down
If GI side effects are interfering with eating, hydration, or daily life, hold at your current dose for another 2–4 weeks instead of stepping up. Many patients reach goal-zone weight loss at 5 mg or 7.5 mg and never need 15 mg.
When to consider going higher
If you've been at your current dose for 8+ weeks with <0.5% weight loss per week and acceptable tolerability, talk to your prescriber about stepping up. Trial data shows clear dose-response — higher doses produce more weight loss on average.
Missing a dose during titration
If you're less than 4 days late: take it as soon as you remember and resume your weekly schedule. If you're more than 4 days late: skip the missed dose and take your next dose on your regular day. Don't double up. Long gaps (>2 weeks) may require restarting at a lower dose.

How long should I stay on each Zepbound dose?

Do I have to reach 15 mg?

What if I miss multiple Zepbound doses in a row?
