Zepbound starts at 2.5 mg and rises by 2.5 mg steps to maintenance doses of 5, 10 or 15 mg; Ozempic starts at 0.25 mg with maintenance of 0.5, 1 or 2 mg. In SURPASS-2, tirzepatide lowered A1C and weight more than semaglutide 1 mg in type 2 diabetes; in SURMOUNT-5, tirzepatide produced 20.2% versus 13.7% weight loss at 72 weeks against Wegovy-dose semaglutide. Both trials were Lilly-funded. Both drugs share stomach side effects and a thyroid C-cell tumor boxed warning.
